RN Case Manager / Social Worker - Full Time

About the role

Full-time RN Case Manager or Social Worker to oversee patient care from admission to discharge within a 170-bed acute care hospital.

Responsibilities

• Implement case management and social work processes: assessment, planning, collaboration, intervention, monitoring, and evaluation
• Review care for appropriateness
• Document assessments within 48 hours
• Ensure safe discharge planning
• Collaborate with healthcare team members to facilitate patient transitions
• Provide disease management education

Requirements

• Current Nevada Registered Nurse or Social Worker License
• At least five years of nursing experience
• Two years of utilization review or case management experience (preferred)
