Job Description & Requirements RN Case Manager or Social Worker StartDate: 11/2/2026 Pay Rate: $1800.00 - $2500.00 Position Summary RN Case Manager or Social Worker POSITION DUTIES Discharge planning coverage for ER/ICU/Med Surg units. This does not include Utilization Management Minimum Required Qualifications 3 or more years of Hospital Case Manager experience Case Management Certification Preferred But Not Required LENGTH OF ASSIGNMENT 13 Weeks SHIFT / HOURS PER WEEK Days, M-F 8am-4:30pm (every fourth weekend) SYSTEMS Cerner START DATE 11/2/2026 Facility Location Springfield riverfront provides wonderful opportunities for bikers, runners, walkers or those who like to rollerblade.
