RN Case Manager / Nursing Supervisor - Home Health Agency - Part-Time or PRN
This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews.
Responsibilities
- Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
- Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- Document, communicate, and coordinate patient status and changes in condition to the care team.
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties as required.
Requirements
- Minimum of one (1) year of nursing supervisory experience; home care experience preferred.
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse, with an active RN License in the State of Ohio.
- CPR and First Aid Certifications required (can be obtained prior to hire date).
- 1 year experience in case management and home health required; 2+ years of home health experience highly preferred.
- Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
- Strong background in nursing, patient care, and confidentiality.
- Strong attendance and dependability track record.
- Familiarity with HIPAA regulations and medical terminology.
- Current Driver’s License and reliable transportation.
- Must have experience with OASIS and NON-OASIS assessments.
- Desirable character traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly, respectful, and positive.
Service Area
Primary service area is centralized in Highland County, OH, with typical travel distances in a 10-60 minute radius, and occasional travel up to 90 minutes. Option for travel up to or beyond a 2-hour radius is available if additional hours are desired (part-time or full-time), which may include Clermont, Hamilton, Clinton, Brown, Adams, and other counties.
Schedule
Flexible and customizable. Typical hours are Monday through Friday (weekdays) during the day (8am - 5pm). No on-call requirements or overnight shifts. Weekend, overtime, respite, and PRN services are offered when available but are not required.
This is a PRN or part-time position with long-term employment opportunities. Full-time option is available if willing to travel up to or beyond 2+ hours as needed. PRN schedule varies week to week, including some weeks with low or no hours.
Pay
Hourly Wage: $35 - $40/hour, paid weekly on Fridays. Full-time RNCMs are eligible for quarterly bonus opportunities.
