RN Case Manager / Nursing Supervisor - Home Health Agency - Part-Time or PRN

About the role

Provide comprehensive care coordination for patients in home health settings. Manage multiple patients simultaneously, ensuring adherence to healthcare protocols and patient safety.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate with healthcare providers to ensure continuity of care
• Monitor patient progress and adjust care plans as necessary
• Ensure compliance with state and federal regulations
• Communicate effectively with patients, families, and other healthcare professionals

Requirements

• Bachelor’s degree in Nursing or related field
• Current RN license in Ohio
• At least 2 years of clinical experience in home health or related field
• Strong organizational and communication skills

Qualifications

• Experience with electronic health records (EHR)
• Knowledge of home health agency policies and procedures
• Ability to work flexible hours including evenings and weekends
