RN Case Manager - Home Health Agency (PRN or Part-Time: Weekdays, Daytime)
About the role
This position will be hired as PRN or Part-Time only. Additional Part-Time hours are also available with a 2-hour travel radius to "Secondary" locations listed below. Full-Time may be available in the future but not at this time and is not guaranteed.
This is a regional leadership role within our agency, requiring travel as needed throughout Dayton and Metro-Dayton areas where clients and caregivers reside. This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate).
For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.
Responsibilities
- RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
- RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- RN documents/communicates/coordinates patient status/change in condition to care team.
- RN promotes personal safety and a safe environment for clients and coworkers.
- Willing to travel surrounding counties.
- Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.
Requirements
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
- Minimum of 1 year of experience in home health agency or equivalent setting.
- CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
- Experience with OASIS and NON-OASIS assessments is also required.
- Strong background in nursing, patient care, and confidentiality.
- Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
- Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology.
- Willingness and ability to perform required physical demands (see below)
- Consent + Ability to pass a State & Federal background check (including fingerprints)
- 18yo + active Driver's License / State ID + US Work Authorization
- Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).
