About the role
Provide comprehensive care coordination for patients in home health settings. Manage patient care plans, coordinate services, and ensure compliance with regulatory standards.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate services including therapy, medication management, and social services
- Communicate with patients, families, and healthcare providers
- Ensure all services are provided in accordance with regulatory requirements
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- Experience in home health care setting
- Strong communication and organizational skills
