RN Case Manager - Home Health Agency (PRN or Part-Time: Weekdays, Daytime) | Dayton / Dayton Metro Area - SW OH
Responsibilities
- Assess patient needs and develop care plans
- Coordinate with healthcare providers to ensure continuity of care
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients and families to provide support and education
- Ensure compliance with regulatory standards and agency policies
Requirements
- Bachelor’s degree in Nursing (BSN)
- Current RN license in Ohio
- At least 1 year of home health nursing experience
- Strong communication and interpersonal skills
- Proficient in computer systems and software
Qualifications
- Excellent organizational and time management skills
- Ability to work independently and as part of a team
- Knowledge of home health agency operations and regulations
