$20,000.00 New Hire Sign-On Bonus! Monday – Friday, 8:00 AM – 5:00 PM (plus holiday, weekend, and on-call rotation). Positions available in:
- Zone 1 – Waukee, Grimes, Johnston
- Zone 2 – Altoona, Pleasant Hill, Ankeny, Polk City
- Zone 3 – Downtown and 50317 area
- Zone 5 – South Des Moines, Indianola, Mitchellville
As a RN Case Manager – Home Health with UnityPoint at Home, you will deliver one-on-one care to patients with full autonomy over your role. The RN Case Manager – Home Health applies clinical nursing knowledge, physical assessment, teaching, and procedural skills to deliver high-quality patient care in the patient’s residence. You will develop and oversee the home health plan of care under physician direction, ensure appropriate interdisciplinary involvement, and deliver care consistent with clinical best practices to achieve improved outcomes and exceptional patient experiences.
Responsibilities
- Conduct home visits for patients requiring home health nursing services and assume responsibility for ongoing interdisciplinary patient assessment.
- Perform initial and comprehensive assessments on all patients in accordance with agency policies and procedures.
- Conduct nursing assessments at each visit per agency policies and procedures.
- Ensure quality and safe delivery of services to patients according to the care plan.
- Use equipment and supplies safely, effectively, and efficiently.
Qualifications
- Education: Graduate of a State Board-approved program for Registered Nurses.
- Licensure/Certifications:
- Valid RN license in the state(s) where providing care.
- Valid licensed driver with automobile insurance per state/organizational requirements.
- Mandatory Reporter: Child & Dependent Adult Abuse.
- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider card with recertification.
- Minimum Experience: One year of nursing experience caring for a similar patient population.
- Other: Ability to use usual and customary equipment required to perform essential functions of the position.
