RN Case Manager - Home Health

About the role

The RN Case Manager provides primary home care for 25-35 patients, assessing needs, developing care plans, and coordinating multidisciplinary services to ensure positive outcomes. They supervise LPNs and Home Health Aides, overseeing care delivery from admission to discharge, and adjust plans based on patient progress.

The role includes patient and caregiver education to promote self-care, effective communication with physicians, and adherence to regulatory guidelines such as CMS, State, and Joint Commission standards.

Responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate multidisciplinary services to ensure positive patient outcomes.
• Supervise LPNs and Home Health Aides, overseeing care delivery from admission to discharge.
• Adjust care plans based on patient progress and evolving needs.
• Provide education to patients and caregivers to promote self-care.
• Communicate effectively with physicians and other healthcare providers.
• Ensure adherence to regulatory guidelines (CMS, State, Joint Commission).
• Maintain accurate and timely documentation.
• Participate in quality improvement activities.
• Support staff development and training initiatives.

Requirements

• Associate or Bachelor’s degree in Nursing.
• Active RN licensure (single or multi-state).
• BLS certification.
• Valid driver’s license with insurance.
• Minimum of one year of Medical/Surgical nursing experience.
• Strong critical care skills.
• Excellent communication, organizational, and problem-solving abilities.

Qualifications

Knowledge of home health operations, reimbursement processes, and OASIS is preferred.
