About the role
Provide coordinated primary care to 25-35 homebound patients, addressing physical, emotional, developmental, psychosocial, and educational needs.
Responsibilities
- Assess, develop, and manage patient care plans.
- Supervise LPNs and Home Health Aides.
- Ensure effective communication with the care team.
- Participate in process improvements.
- Provide patient and caregiver education to promote self-care and independence.
- Maintain compliance with regulatory guidelines.
- Complete documentation accurately.
- Participate in performance improvement activities.
- Support staff education.
- Adhere to productivity standards.
- Perform weekend and call duties as required.
Requirements
- Graduate of an approved nursing program.
- Valid RN license (single state or multi-state).
- BLS certification.
- At least 1 year of Medical/Surgical nursing experience; home health and ICU experience preferred.
- Strong critical care assessment skills.
- Leadership, problem-solving, and organizational skills.
- Excellent communication skills.
- Proficiency with computer systems.
- Knowledge of home health operations and regulations.
