RN Case Manager - Home Health

About the Role

We are seeking an experienced Home Care RN/Field Case Manager to provide skilled nursing visits and coordinate patient care in a community-based setting.

Responsibilities

• Assess patients, develop and manage care plans.
• Perform complex wound care, manage central lines and PICC lines, administer infusions, and monitor devices such as catheters, drains, and tracheostomies.
• Educate patients and families to promote health, independence, and wellness.
• Collaborate with an interdisciplinary team, direct licensed practical nurses and home health aides, and ensure regulatory compliance in documentation and care standards.
• Conduct OASIS assessments, manage case documentation, and communicate with physicians and care team members regarding patient status and updates.

Requirements

• Valid IL RN license, BLS certification, and a valid driver’s license with proof of auto insurance.
• Minimum of 1 year nursing experience.
• Knowledge of Medicare home care benefits, regulatory requirements, and documentation standards.

Skills

• Strong assessment, communication, and organizational skills.
• Ability to work independently.
