RN Case Manager - Home Health

About the role

The RN Case Manager provides primary care to 25-35 home-based patients, addressing physical, emotional, developmental, psychosocial, and educational needs.

Responsibilities

• Assess patient needs and develop/adjust care plans.
• Supervise LPNs and Home Health Aides.
• Coordinate multidisciplinary services to ensure positive outcomes.
• Communicate with physicians, patients, families, and team members.
• Ensure accurate documentation and adherence to regulatory guidelines.
• Participate in performance improvement activities.
• Provide patient education and staff training.
• Contribute to quality improvement initiatives.

Requirements

• Nursing degree from an accredited program.
• RN licensure (multi-state preferred).
• BLS certification.
• At least one year of Medical/Surgical nursing experience.

Qualifications

Preferred background in home health and ICU.

Skills

• Strong clinical, leadership, organizational, and communication abilities.
• Proficiency in home health operations, regulatory compliance, and documentation systems.
