About the role
Provide personalized home-based care to patients recovering from illnesses or managing chronic conditions. Collaborate with physicians and oversee LPNs, CNAs, and aides to deliver comprehensive care. Conduct patient assessments, document progress, and communicate changes to healthcare providers. Educate patients and their families on care plans, medications, therapies, and safety measures. Ensure care goals are achieved and coordinate patient discharge processes.
Qualifications & Skills
- Active, unrestricted RN license in Michigan (BSN not required)
- Minimum of one year of clinical RN experience
- Current CPR certification
- Reliable transportation with valid driver’s license and insurance
