RN Case Manager - Home Health

Responsibilities

• Provide home-based care to patients with various health conditions, often after hospital or rehab discharge
• Collaborate with physicians and oversee LPNs, CNAs, and aides to deliver patient care
• Perform patient assessments, document progress, and communicate changes to physicians
• Educate patients and families on care plans, medications, therapy, diet, and safety
• Coordinate discharge planning and ensure care goals are met

Requirements

• Active, unrestricted RN license in Michigan
• Minimum of one year of clinical nursing experience
• Current CPR certification
• Reliable transportation, valid driver’s license, and insurance
