RN Case Manager- Home Health

About the role

The Home Health and Hospice RN Case Manager assesses, plans, implements, and evaluates patient care in order to achieve optimum patient outcomes for patients as appropriate to service area.

Responsibilities

• Completes age-specific admission and ongoing assessments, documents, and uses them to develop a plan of care.
• Nursing care and provision of individualized care for patients.
• Utilizes delegation and critical thinking skills to provide quality service through a team-oriented, collaborative approach to patient care.
• Interacts with patients, families, visitors, and all members of the health care team to develop, coordinate, and manage the plan of care.
• Manages and oversees delivery of patient care such as but not limited to:
• Maintains patient, staff, and customer confidentiality.
• Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient’s status.
• Interprets the appropriate information needed to identify each patient's requirements relative to their age-specific needs and provides care according to departmental policies and procedures.
• Works in a fast-paced environment with the ability to effectively prioritize and demonstrate multi-tasking skills.
• Handles routine issues, follows up to conclusion on matters, while referring only the most complex or higher-level issues and problems to the Manager, Team Lead, or Supervisor.

Requirements

• Graduate of accredited Registered Nurse Program.
• Currently licensed as Registered Nurse in the state of Virginia or approved compact state.
• Valid driver’s license, automobile insurance, and personal vehicle for making home visits.

Qualifications

• Keyboarding skills required for automated computer systems, including electronic medical records.
• Knowledge of growth and development to appropriately care for patients served.

Skills

• Strong critical thinking skills necessary to interpret laws and regulations related to CHCS and Carilion Clinic.
