About the Role
Provide personalized, home-based care to patients recovering from illnesses, injuries, or managing chronic conditions.
Responsibilities
- Collaborate with physicians, oversee LPNs, CNAs, and aides to deliver comprehensive patient care.
- Conduct patient assessments, document progress, and communicate changes to healthcare providers.
- Educate patients and families on care plans, medications, therapies, and safety measures.
- Coordinate patient discharge and ensure care goals are achieved.
Requirements
- Active, unrestricted RN license in Michigan.
- Minimum of one year of clinical nursing experience.
- Current CPR certification.
- Reliable transportation, valid driver’s license, and insurance.
