RN Case Manager - Home Health

Start Date: 8/31/2026 • Pay Rate: $1,800.00 - $2,700.00

About the role

Collaborate with the patient, their family, and the physician to develop and regularly update an individualized care plan, coordinating services through ongoing interdisciplinary communication. Assess the patient’s clinical, functional, and emotional status, documenting progress and reporting significant changes to the care team. Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions, while using each interaction as an opportunity for meaningful health counseling.

Responsibilities

• Assess patient’s clinical, functional, and emotional status

• Educate patients and caregivers on self-care techniques, medications, diet, and physician-ordered instructions
• Provide meaningful health counseling during interactions

Requirements

• Compact or VT & NH unencumbered RN license
• Graduate of an accredited school of nursing
• BLS certification
• 2 years of experience in Home Health settings
