About the role
The RN Case Manager - Home Health position at AMN Healthcare is responsible for collaborating with patients, families, and physicians to develop and update individualized care plans. This role involves assessing patients' clinical, functional, and emotional status, educating them on self-care techniques, and ensuring regular communication with the care team.
Responsibilities
- Collaborate with the patient, their family, and the physician to develop and regularly update an individualized care plan, coordinating services through ongoing interdisciplinary communication.
- Assess the patient’s clinical, functional, and emotional status, documenting progress and reporting significant changes to the care team.
- Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions, while using each interaction as an opportunity for meaningful health counseling.
Requirements
- Compact or VT & NH unencumbered RN license
- Graduate of an accredited school of nursing
- 2 years of experience in Home Health settings
