RN Case Manager - Full-Time

We are seeking a qualified Registered Nurse to provide safe, personalized discharge planning and utilization management services aimed at achieving quality healthcare outcomes and maximizing reimbursement. Responsibilities Include Collaborating proactively with patients, families, and healthcare teams to develop discharge plans.Assessing social and physical factors affecting discharge and utilizing community resources to meet post-discharge needs.Ensuring timely and proficient management of patient utilization to support optimal care and reimbursement. Qualifications Graduate of an accredited nursing school.Minimum of three years clinical experience; two years in Utilization Review, Management, or Case Management preferred.Current Nevada Registered Nurse license.Knowledge of social, physical, and community resources affecting patient discharge.Proficiency in computer applications for data management and documentation.Ability to provide age-appropriate care based on patient needs.
