RN Case Manager - Full-Time

Role Overview: The position is responsible for delivering quality healthcare outcomes by establishing safe, individualized discharge plans and providing proficient utilization management services to maximize reimbursement. Key Responsibilities Collaborate proactively with patients, families, and healthcare teams to ensure effective discharge planning.Manage utilization review and case management processes to optimize patient care and reimbursement.Assess social and physical factors affecting discharge and coordinate community resources to meet post-discharge needs. Qualifications & Skills Graduate of an accredited nursing school.Minimum three years of clinical experience, with two years in Utilization Review, Management, or Case Management preferred.Current RN license in the state of practice.Knowledge of factors influencing functional status at discharge and community resources.Proficiency in computer applications including word processing, spreadsheets, and data management.Ability to provide age-appropriate care/services.
