RN Case Manager - Full-Time

Role Overview: Responsible for ensuring quality healthcare outcomes by establishing safe, individualized discharges and providing proficient utilization management services to maximize reimbursement. The role involves proactive collaboration with patients, families, and healthcare teams to coordinate care effectively. Key Responsibilities Facilitate safe discharge planning tailored to patient needs.Provide timely utilization review and management services.Collaborate with multidisciplinary teams to optimize patient care and reimbursement. Qualifications & Skills Graduate of an accredited nursing school.Minimum three years of clinical experience; two years in Utilization Review, Management, or Case Management preferred.Knowledge of social and physical factors affecting discharge and community resources.Current Nevada Registered Nurse license.Computer proficiency in word processing, spreadsheets, and data management.Ability to provide age-appropriate care/services.
