RN Case Manager - Full-Time

We are seeking a qualified Registered Nurse to provide quality healthcare outcomes through safe, individualized discharge planning and proficient utilization management services. Key Responsibilities Include Collaborating proactively with patients, families, and healthcare teams to optimize reimbursement and patient care.Establishing safe discharge plans considering social and physical factors affecting functional status.Utilizing community resources to meet post-discharge clinical and social needs. Qualifications Graduate of an accredited nursing school.Minimum of three years clinical experience; two years in Utilization Review, Management, or Case Management preferred.Current Nevada Registered Nurse license.Knowledge of social and physical factors impacting discharge and familiarity with community resources.Proficiency in computer applications including word processing, spreadsheets, and data management.Ability to provide age-appropriate care based on patient demographics.
