RN Case Manager - Full-Time

We are seeking a registered nurse with a minimum of three years' clinical experience, including two years in Utilization Review, Management, or Case Management. Key Responsibilities Include Establishing safe, individualized discharge plans to promote quality healthcare outcomes.Providing proficient, timely utilization management services to maximize reimbursement.Collaborating proactively with patients, families, and healthcare teams to ensure appropriate care and resource utilization.Assessing social and physical factors affecting discharge and connecting patients with community resources to meet post-discharge needs. Qualifications Required Graduate of an accredited nursing school.Current Nevada Registered Nurse license.Knowledge of factors influencing discharge planning and community resources.Proficiency in computer programs including word processing, spreadsheets, and data management.Ability to provide age-appropriate care for diverse patient populations.
