RN Case Manager - Full-Time

About the role

We are seeking a registered nurse to provide quality healthcare outcomes by establishing safe, individualized discharge plans and delivering timely utilization management services. The role involves proactive collaboration with patients, families, and healthcare teams to ensure maximum reimbursement and optimal patient care.

Qualifications

• Graduate of an accredited nursing school
• Minimum three years of clinical experience, with two years in Utilization Review, Management, or Case Management preferred
• Knowledge of social and physical factors affecting discharge and community resources
• Current RN license in the state of practice
• Proficiency in computer programs for word processing, data management, and reporting
• Ability to provide age-appropriate care based on patient needs
