RN Case Manager - Full-Time

Role And Responsibilities Provide quality healthcare outcomes by establishing safe, individualized discharge plans and delivering proficient utilization management services to maximize reimbursement. Collaborate proactively with patients, families, and healthcare teams to ensure effective care transitions and meet clinical and social needs. Qualifications And Skills Graduate of an accredited nursing school.Minimum of three years clinical experience, with two years in Utilization Review, Management, or Case Management preferred.Knowledge of social and physical factors affecting discharge and community resources.Proficiency in computer programs including word processing, spreadsheets, and data management.Current Registered Nurse license in Nevada.Ability to provide age-appropriate care/services to patients. Key Duties Coordinate discharge planning to ensure safe, timely patient transitions.Conduct utilization review to optimize reimbursement.Collaborate with healthcare teams, patients, and families to meet care goals.
