RN Care Manager - Exempt

Position Overview: The Registered Nurse Care Manager is responsible for assessing participants' healthcare needs, developing and implementing personalized care plans in collaboration with an interdisciplinary team (IDT), and providing nursing interventions across various settings including clinic, center, and home. Key Responsibilities Assess, plan, and coordinate participants’ care, including home services and medical interventions.Monitor acute and chronic health conditions, ensuring timely follow-up and communication with providers.Reconcile medications, review test results, and educate participants and caregivers on self-care, medications, and safety.Delegate tasks to aides, supervise their performance, and evaluate their competence.Participate in team meetings, quality improvement initiatives, and document all care activities accurately and timely.Support and collaborate with the multidisciplinary team to ensure comprehensive, patient-centered care.Adhere to all safety, privacy, and infection control protocols. Qualifications & Skills State RN license and BLS certification (within 90 days of hire).Preferred Bachelor of Science in Nursing and at least 1 year of RN experience, especially with elderly or frail populations.Experience in home or long-term care is a plus.Strong communication, critical thinking, and teamwork skills. Additional Requirements Reliable transportation, valid driver's license, and up-to-date immunizations.Successful completion of background checks and medical clearance.
