RN Care Manager - Exempt

About the role

Join our team as a Registered Nurse Care Manager and make a meaningful difference in the lives of seniors.

• Assess participants' care needs and develop personalized plans of care in collaboration with an interdisciplinary team (IDT)
• Provide nursing interventions and coordinate services across clinic, center, and home settings
• Delegate tasks to aides and monitor ongoing care outcomes
• Educate participants and families on self-care, medications, and safety
• Participate in team meetings, quality improvement initiatives, and utilization reviews
• Ensure compliance with policies, safety guidelines, and confidentiality standards

Qualifications

• State RN License and BLS certification
• Bachelor of Science in Nursing preferred
• Minimum one year of RN experience providing care, preferably with elderly or frail populations
• Experience in home or long-term care is a plus
