RN Care Manager - Exempt

About the role

We are seeking a Registered Nurse Care Manager to provide comprehensive nursing assessments, develop and implement personalized care plans, and coordinate services for older adults in a community-based program. Responsibilities

• Assessing participants' health needs and providing nursing interventions in various settings, including clinics, homes, and centers.
• Collaborating with an interdisciplinary team (IDT) to develop and revise individualized care plans.
• Delegating tasks to aides and monitoring medication adherence, ensuring accurate documentation and follow-up.
• Educating participants and caregivers on self-care, medications, and safety practices.
• Participating in team meetings, quality improvement initiatives, and utilization reviews.
• Maintaining confidentiality, following safety protocols, and adhering to program policies.

Qualifications and Skills

• State RN license and BLS certification.
• Bachelor of Science in Nursing preferred.
• Minimum of one year experience working with elderly or frail populations; experience in home or long-term care is advantageous.
• Strong communication, teamwork, and assessment skills.
• Reliable transportation and ability to pass background checks and immunization requirements.
