RN Care Manager - Exempt

Role Overview

The Registered Nurse (RN) Care Manager assesses participants' care needs, provides nursing interventions, and evaluates care outcomes. Working collaboratively with the interdisciplinary team (IDT), the RN develops and implements personalized care plans, delegates tasks to aides, and ensures coordination across clinic, home, and facility settings.

Responsibilities

• Deliver high-quality clinical care and participate in 24/7 triage for participant needs.
• Assess, plan, and coordinate home and healthcare services, ensuring timely follow-up and medication reconciliation.
• Educate participants and families on self-care, medications, and safety protocols.
• Collaborate with the IDT to revise care goals and strategies, and participate in team meetings.
• Support quality improvement initiatives and evaluate staff competency.

Qualifications & Skills

• State RN license and BLS certification required.
• Bachelor of Science in Nursing preferred.
• Minimum of one year RN experience, preferably with elderly or frail populations.
• Experience in home or long-term care is advantageous.
• Strong communication, collaboration, and organizational skills.
