About the Role

At PeaceHealth, Care Management is more than coordinating discharge plans. It’s about advocating for patients, supporting families through complex healthcare journeys, and ensuring every patient receives the right care, in the right setting, at the right time. The RN Care Manager coordinates and facilitates patient care planning from admission through discharge, ensuring patients receive appropriate services and resources throughout their healthcare journey.

Responsibilities

• Evaluate patient admissions and determine appropriate levels of care using established clinical criteria.
• Develop, implement, and oversee individualized discharge and transition plans.
• Assess patients' clinical, psychosocial, environmental, financial, and post‑acute care needs.
• Coordinate services and community resources to ensure smooth transitions across the continuum of care.
• Collaborate with physicians and interdisciplinary teams to optimize patient outcomes.
• Facilitate care conferences and interdisciplinary meetings.
• Partner with insurance providers and third‑party payors to secure authorizations and minimize barriers to care.
• Monitor utilization and admission status to ensure compliance with established guidelines.
• Advocate for patients and families while supporting safe, effective discharge planning.
• Participate in quality improvement initiatives and contribute to enhanced patient satisfaction outcomes.
• Support palliative care coordination and family‑centered care discussions when appropriate.

Requirements

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the state of practice.
• Basic Life Support (BLS) certification upon hire.
• Minimum of three years of clinical nursing experience in an acute care hospital.
• Care Management experience required.
• Equivalent experience in skilled nursing facilities, outpatient case management, home health, or hospice may be considered.

Qualifications

• Bachelor of Science in Nursing (BSN) – preferred.
• Master of Science in Nursing (MSN) – preferred.
• Utilization Management or Quality Management experience – preferred.
• Specialty certification – preferred.
• Knowledge of third‑party reimbursement processes – preferred.

Pay

Salary Range: $45.78 – $82.98 per hour, depending on experience.

Schedule

Full‑Time, 0.90 FTE. Day shift options: 07:00 – 15:30 or 08:30 – 17:00.
