Care Manager RN - Care Management 0.9 FTE Day

About the role

Care Manager RN Provides comprehensive care management services in an acute care setting, including screening for post-acute needs, transition planning, care coordination, patient education, community referrals, and ongoing monitoring of care plans to ensure regulatory compliance and effectiveness.

Responsibilities

• Assess patient needs across the continuum of care
• Collaborate with healthcare teams and community resources
• Reassess and adjust care plans as needed
• Ensure regulatory and organizational standards are met

Qualifications

• Nursing degree/diploma required at hire; Bachelor's in Nursing preferred or obtained within 3 years
• Oregon RN license upon hire
• Minimum 2 years of acute care or community-based experience
• At least 1 year experience with electronic medical records and care management, discharge planning, or related fields

Skills

• Strong care coordination, assessment, communication, and collaboration skills
• Knowledge of healthcare regulations and documentation systems
