About the role
This is a full-time RN Care Manager position focused on care management within an acute care setting. The role involves screening for post-acute needs, transition planning, care coordination, patient education, community referrals, and ongoing monitoring of care plans.
Responsibilities
- Collaborate with care teams
- Ensure regulatory compliance
- Reassess patient needs
- Facilitate seamless patient progression through the continuum of care
Qualifications & Skills
- Active Oregon RN license
- Nursing diploma or degree
- At least 2 years of acute or community-based care experience
- Experience with electronic health records and care management, discharge planning, or related fields
- Preferred: Bachelor's degree in nursing
- Preferred: Certification as a Case Manager
