RN - Behavioral Health Clinic

Role Overview: This nursing position involves joining a collaborative outpatient behavioral health team, providing care for patients aged 5 and up. Responsibilities include phone triage, patient education, and support, as well as multi-tasking with returning calls, portal messages, and assisting during clinics. Key Responsibilities Provide and facilitate competent healthcare for patients and families, ensuring adherence to standards and accurate documentation.Offer health and wellness education to individuals and groups.Act as a preceptor and support staff and student development.Lead quality improvement initiatives and assist in developing departmental orders.Coordinate care for patients aged 0-150, perform telephone triage for acute and chronic needs, and assist with procedures such as wound care and lesion removal.Administer immunizations and medications, and provide education for newly diagnosed diabetes patients. Qualifications & Skills Licensed Registered Nurse (RN) with a current Illinois license.Basic Life Support (BLS) certification within 30 days.High school diploma or GED.Strong communication, multitasking, and patient care skills. Additional Requirements: Ensuring comprehensive documentation, supporting care coordination, and participating in quality improvement efforts.
