Residential LPN

About the role

LifeStream is hiring full and part-time Licensed Practical Nurses (LPNs) to ensure the medical needs of individuals with disabilities are met. You will assist with daily living skills, including emotional and social support, in their homes and in the community. No prior experience within the agency is required.

Responsibilities

• Assess individuals’ medical regimes to ensure referrals for health services that maximize health and safety.
• Ensure consumers receive services to achieve set goals and maximize physical and emotional well-being.
• Provide direct care duties to maintain the highest level of care standards.
• Maintain medical charts, medication ordering regimes, and medical supply inventory.
• Assess consumer complaints and overall well-being daily.
• Review doctors’ orders for accuracy.
• Adhere to DPH regulations for medication storage, administration, and destruction.

Requirements

• Current LPN license in the Commonwealth of Massachusetts.
• Valid driver’s license and access to a vehicle for work purposes.
• Ability to pass employment screening process (CORI, fingerprinting, negative drug screen).
• Commitment to the philosophy of community inclusion.
• Familiarity with the needs of individuals with developmental disabilities and brain injury is preferred.

Pay

Starting pay is $36/hour, with weekend shift differentials available.

Schedule

Full-time and part-time positions available. Day and evening shifts offered.
