About the role
The Residential LPN is responsible for ensuring optimal medical care for group home residents and assisting in establishing sound medical procedures.
Responsibilities
- Coordinates scheduling and attends medical appointments for residents and provides ongoing communication with physicians regarding resident’s health care.
- Makes sure medication administration is accurate, comparing physician orders with Medication Administration Record and prescription labels.
- Ensures that all physician orders are completed and communicated effectively, providing follow-up care and observation for residents' health issues, accidents, and seizures.
- Communicates with the pharmacy to ensure that prescribed medications are delivered and available in a timely manner.
- Maintains detailed records of nursing contacts for each resident, including observations and chart reviews.
- Maintains residents' medical files and documentation in accordance with State Board of Health and Medicaid requirements.
- Updates electronic medication administration records in real time as new medication orders are received.
- Schedules and assists in administering vaccines/tests, including HEP-B and TB, and quarterly physical and medication reviews.
- Trains agency personnel on various medical techniques, including universal precautions, signs and symptoms of illnesses, and seizure disorder management and other medical conditions specific to individuals served.
- Provides initial and ongoing training in state-mandated medication administration curriculum.
- Responds to medical emergencies and concerns that arise outside of regular working hours.
- Reports on progress to each individual's Interdisciplinary team and participates in agency committees such as Human Rights Committee, Safety Committee, etc.
- Performs related duties as assigned, ensuring the best interest of both the agency and the clients' welfare.
Qualifications
- Certified as a Licensed Practical Nurse (LPN) and possesses a current Indiana license in nursing.
- Thorough knowledge of and practical application of PDR, State Board of Health, EDS/Medicaid Review criteria, and other applicable regulations regarding health care.
- Ability to obtain and maintain certification in Core A & B (state-mandated medication administration curriculum) and TB skin test administration within required timeframes.
- Ability to work independently and with others in a team environment, often under time pressure.
- Ability to read and interpret physician orders and lab reports, while maintaining confidentiality of information and reports.
- Ability to communicate effectively with other agency personnel, residents, and their families/guardians, as well as health care providers, including sensitivity to professional ethics, gender, cultural diversities, and disabilities.
- Ability to perform physical examinations of residents, including grasping and carrying objects, bending at the waist, crouching, kneeling, and lifting residents.
- Ability to respond quickly, rationally, and decisively to emergency situations, despite potential exposure to potentially violent/irritant individuals and situations involving potential physical harm to self and others.
- Ability to work irregular and/or extended hours, responding to medical emergencies and concerns outside of regular working hours.
- Compliance with all personnel policies and rules of the organization.
- Valid driver's license and a demonstrated safe driving record.
