About the Role
Under the direction of facility leadership, this position is responsible for the planning and coordination of direct resident care services. Serves as a core member of the interdisciplinary client care team with the necessary authority to assess, plan, evaluate, and teach in patient and staff situations. Carries out administrative and supervisory responsibilities as delegated by facility leadership.
Responsibilities
- Provide clinical leadership and supervision to nursing staff and care teams.
- Oversee daily operations to ensure high-quality resident care.
- Coordinate care plans and ensure clinical excellence and compliance.
- Collaborate with interdisciplinary teams to support resident well-being.
- Serve as a resource, mentor, and leader for staff.
- Promote a safe and supportive environment for residents and team members.
- Coordinate admissions, discharges, and transfers to and from the facility.
- Provide resident care by reviewing physician prescriptions and communicating the nursing care plan to the healthcare team.
- Develop nursing care plans by obtaining medical histories, examining residents, and documenting conditions.
- Participate in interdisciplinary team meetings for development, review, and revision of care plans.
- Evaluate resident care by adhering to therapeutic standards and measuring health outcomes.
- Supervise medical records for accuracy, completeness, and compliance with regulations.
- Participate in quality improvement (QI) activities.
- Develop expertise in Medicare, MDS assessment, Case Mix, payer sources, and reimbursement issues.
- Maintain financial integrity of the unit via accurate patient charges.
- Review, update, and standardize forms and maintain current procedure manuals and policies.
- Serve as a resource person to healthcare professionals, nursing students, and community members.
- Interview, hire, develop, train, and retain employees; plan, assign, and lead work.
- Appraise performance, address complaints, and resolve problems.
Requirements
- Registered Nurse (RN) with an active license in the state of practice (upon hire).
- Ability to work the specified hours, including visual acuity and effective communication skills.
