About the role

Under the direction of facility leadership this position is responsible for the planning and coordination of direct resident care services. Serves as a core member of the interdisciplinary client care team. Has the necessary authority to assess, plan, evaluate and teach in patient and staff situations. Carries out administrative and supervisory responsibilities as delegated by facility leadership.

Responsibilities

  • Coordinates admissions, discharges and transfers to and from the facility.
  • Provides resident care by reviewing physician prescriptions; communicating the nursing care plan and all pertinent information to other members of the health care team; coordinating with other healthcare providers; providing physical, emotional, psychological, and spiritual support to residents, families, and friends; conferring with physicians.
  • Develops nursing care plans by obtaining medical histories; examining residents; documenting residents physical condition, psychological status, and influencing social factors; conferring with family and friends; developing a nursing diagnosis; establishing nursing care objectives; defining nursing interventions, schedules, and timetables.
  • Participates in interdisciplinary team meetings for development, review and revision of plan of care for resident and family. Initiates, participates and coordinates additional meetings as necessary to address crisis matters and provide support for team members.
  • Evaluates resident's care by adhering to therapeutic standards; measuring health outcomes against resident care standards; reviewing resident's records; noting changing conditions and needs; re-evaluating care objectives; modifying nursing care plan.
  • Serves as a resource person to other healthcare professionals, nursing students and community members.
  • Supervises medical record assuring accuracy, completeness and compliance with State and Federal regulations, legal and ethical standards.
  • Participates in QI activities.
  • Develops expertise in Medicare, MDS assessment, Case Mix, payer sources and reimbursement issues. Maintains financial integrity of the unit via accurate patient charges.
  • Reviews, updates and standardizes forms. Maintains current procedure manual and policies.
  • Serves as a resource person to other healthcare professionals, nursing students and community members.
  • Interviews, hires, develops, trains, and retains employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.

Requirements

The individual must be able to work the hours specified. To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.

Qualifications

  • Registered Nurse (RN) - Board of Nursing
  • An active license in the state of practice
  • Geriatric, medical or surgical experience preferred
