Resident Care Coordinator (Full-Time) - The Reserve at Mills Farm

Responsibilities

  • Provide direct resident services and medication management when needed
  • Responsible for the scheduling and supervision of resident care staff
  • Carefully coordinates and monitors the completion of daily assignments
  • Develops programming that meets the specific needs and abilities of residents residing in Memory Care
  • Ensures that personalized Memory Boxes are in place for each resident
  • Works with Activities Coordinator to provide supervision and support for activities and outings
  • Completes/updates resident service plans to reflect the specific needs/abilities of each resident
  • Demonstrates ability to manage and respond appropriately to resident behaviors
  • Communicates with physicians/other health care providers regarding the resident's health status when appropriate
  • Maintains accurate and complete resident documentation
  • Schedules tests ordered by the physician, assists residents in scheduling medical appointments and transportation
  • Demonstrates competency in all areas of medication administration
  • Maintains current knowledge of state regulations and community policies
  • Affords assistance with orientation and education of resident care staff
  • Responsible for the evaluation and discipline of resident care staff
  • Maintains current knowledge of the community's fire safety procedures, including the correct use of the fire alarm system
  • Able to follow the designated plan of action in the event of a fire or other emergency
  • Conducts/coordinates departmental meetings/trainings sessions
  • Addresses resident/family complaints related to non-clinical issues
  • Demonstrates courteous, polite and friendly attitude with residents, families, visitors and co-workers

Requirements

  • MedTech certification for Adult Care Home required
  • Assisted Living and Memory Care experience required
  • General understanding of and concern for the needs of seniors
  • Aptitude and previous experience with Alzheimer's and memory impaired residents
  • Ability to work in an environment conducive to caring for residents without posing a substantial. safety or health threat to self or others
  • Ability to manage team processes and promote a team environment
