Responsible for

  • Makes and keeps care staff schedule updated as well as finds coverage to fill PTO requests.
  • Reviews assignments with management team.
  • Acts as on-call staff and fills in shifts when requested by the Director of Nursing Services.
  • Reads staff communication records and maintains familiarity with resident records as required.
  • Assists residents with activities of daily living, assistance with medications, treatments and other care while encouraging self-care and independence.
  • Provides resident services as indicated on the resident’s assessment and service plan.
  • Observes problems, concerns, issues with employees and/or residents and communicates them appropriately to Director of Nursing Services.
  • Records resident information as required.
  • Aids in the orientation of new staff members to job responsibilities.
  • Acts as a lead/trainer for care staff.
  • Marks for receipt of physician orders on a daily basis. Follows up with physicians on orders not returned.
  • Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security work, and other emergency procedures.
  • Notifies physician, nurse and / or designated agent(s) of resident status/change as appropriate.
  • Coordinates care with outside agencies.
  • Participates in service plan meetings with residents and families.
  • Safeguards medications to ensure residents do not run out of medications.
  • Ensures all medications are placed in a timely manner.
  • Ensures all doctor’s orders have been transcribed correctly in the MAR (Medication Assistance Record) and ensures all orders have been signed off by the Medication Aide.
  • Completes monthly MAR reviews and follows up on any discrepancies.
  • Attends and participates in required meetings and trainings.
  • Performs other tasks/duties as assigned.

Experience, Qualifications & Skills

  • LPN, CNA, or HCA license.
  • Previous experience in Assisted Living.
  • A high school diploma or equivalent.
  • Ability to read, write, speak and understand English.
  • Maintains a current First Aid and CPR certification.
  • Pass a criminal background and fingerprint check.
  • Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
  • Has compassion and a special interest in working with the senior population.
  • Ability to work a flexible schedule, including weekends, holidays and overtime, to meet the requirements of the position.
  • Ability to relate positively and communicate appropriately with residents, families, community members, volunteers and other employees.
  • Ability to creatively problem-solve in resident care.
  • Maintains clean, neat, comfortable, safe environment for residents, staff, and visitors.
  • Ability to work independently (with minimum supervision).
  • Maintains current Food Handlers Card.
