Essential Position
The Resident Care Coordinator serves as a clinical support lead within the resident care team, assisting the Health Services Manager with daily staffing coordination, scheduling, communication, and resident care operations. This position provides direct resident care as needed, supports medication administration within certification and scope of practice, and serves as a resource for caregivers by helping coordinate workflow and ensuring continuity of care.
Qualification Requirements
- High school diploma or equivalent (GED) required.
- Completion of CNA I training program and valid Nurse Aide I registry listing.
- Completion of the 24-hour Medication Aide training program approved by the NC Board of Nursing.
- Current CPR certification (or ability to obtain within 30 days of hire).
- One year of relevant experience working in long-term care or assisted living, preferred.
- Experience with staff scheduling or care coordination preferred.
Knowledge, Skills And Abilities
- Knowledge of resident care practices, activities of daily living (ADLs), dementia care principles, and person-centered care.
- Knowledge of medication administration practices consistent with state regulations and certification requirements.
- Strong organizational and scheduling skills with the ability to coordinate staffing priorities and respond to changing resident care needs.
- Ability to communicate effectively and professionally with residents, families, healthcare providers, and team members.
- Ability to prioritize multiple responsibilities while maintaining accuracy and attention to detail.
- Critical thinking and sound judgment in recognizing changes in resident condition and communicating concerns appropriately.
- Proficiency with electronic health records, scheduling software, and standard computer applications.
- Ability to foster teamwork, professionalism, and a positive resident-focused environment.
Essential Functions
- Coordinate daily staffing assignments and assist the Health Services Manager with scheduling, shift coverage, call-offs, and staffing adjustments to support quality resident care.
- Provide direct resident care, including assistance with activities of daily living, mobility, dining, and medication administration within certification and scope of practice as needed to support staffing and resident needs.
- Monitor daily care delivery, communicate resident care concerns to the Health Services Manager or Administrator, and assist in ensuring timely follow-up on changes in resident condition.
- Serve as a resource for caregivers by assisting with orientation, mentoring, workflow coordination, and reinforcing established care policies, procedures, and service standards.
- Maintain accurate documentation related to resident care, medication administration, staffing schedules, and other required records in accordance with company policy and regulatory requirements.
- Collaborate with nursing, life enrichment, dining, housekeeping, and other departments to promote continuity of care and enhance the resident experience.
- Assist with admissions, resident transitions, physician appointments, family communication, and other care coordination activities as directed by the Health Services Manager or Administrator.
- Maintain adequate clinical supplies, report equipment or safety concerns promptly, and support a clean, organized, and safe care environment.
- Comply with all company policies, procedures, and applicable laws, including HIPAA, infection prevention standards, medication management requirements, and the Code of Conduct, completing required training and reporting concerns promptly.
