About the role
Merrill Family of Senior Living Communities is hiring a Resident Care Coordinator at Merrill Gardens at Cedar Mill. Responsibilities
- Coordinating resident care services and supporting personalized service planning for assisted living residents.
- Assisting with resident move-ins, assessments, care planning, and transitions of care.
- Monitoring resident health, functional, and psychosocial needs and reporting significant changes to appropriate parties.
- Ensuring resident records, service plans, medication documentation, and caregiver charting are accurate and compliant with company and regulatory standards.
- Coinciding caregiver orientation, training, and ongoing education while supporting team development and performance.
- Assisting with staffing, scheduling, and shift coverage to ensure resident care needs are met.
- Serving as a liaison between residents, families, physicians, and community leadership.
- Providing hands-on resident care support.
Requirements
- A high school diploma or GED required; active CNA/HCA credentials preferred or required in accordance with state regulations.
- 3+ years of experience in assisted living, senior living, or a related healthcare environment.
- Knowledge of resident care planning, documentation, medication management, and assisted living regulations.
- Experience supporting caregiver training, staffing, scheduling, or team leadership.
- Strong communication, customer service, organizational, and problem-solving skills.
- A commitment to providing exceptional resident-centered care while fostering positive relationships with residents, families, and team members.
Qualifications
- Frequent standing, walking, lifting (up to 75 lbs), and assisting with resident care.
- Regular bending, reaching, and physical movement are required throughout the workday.
