Rehab Specialist - Full Time, Weekdays

About the role

Provides rehabilitation services in a community-based setting, supporting individuals with brain, spinal cord, and complex injuries or illnesses.

Responsibilities

• Assess resident needs and design and implement personalized rehabilitation programs.
• Perform skill training, cognitive remediation, job coaching, and behavior management.
• Reinforce appropriate behaviors across residential, community, and vocational environments using feedback, role-playing, and behavior modification techniques.
• Plan, schedule, and evaluate social, emotional, recreational, and vocational activities.
• Maintain accurate clinical documentation and client records.
• Coordinate transportation to activities and collect data related to rehabilitation goals.
• Assist with emergency coverage and crisis intervention as needed.

Requirements

• High school diploma.
• Valid driver’s license.
• Effective problem-solving and communication skills.
• Reliability, compassion, and attention to detail.
• Travel may be required.

Physical demands

Involves heavy exertion, occasionally lifting up to 100 pounds.
