About the Role

The Clinical Manager provides oversight of all patient care services and personnel. This includes coordination and management of patient care, and supervision and coaching of clinical personnel to ensure care and services are delivered appropriately. Additional responsibilities include CHAP/survey readiness, QAPI, patient satisfaction surveys, and complaint reviews.

Responsibilities

• Supervises day-to-day clinical and office operations.
• Available to staff and patients during hours of operation.
• Ensures services comply with professional standards, agency policy and procedure, and state and federal regulatory requirements.
• Serves as back-up support for positions as needed: PCM, CSS, and leader in absence of the Territory Administrator.
• Coordinates referrals and patient care with an interdisciplinary team approach.
• Assures the development, implementation, and updating of individualized plans of care.
• Evaluates the quality of clinical care, ensuring patient needs are continually assessed.
• Collaborates with clinicians and physicians regarding patient transfers or discharges.
• Reviews discharge assessment data, edits/locks discharge OASIS, and handles discipline-only discharges.
• Owns QAPI and participates in QA and performance improvement activities.
• Reviews/investigates incidents, analyzes trends, and implements corrective action plans as needed.
• Handles accreditation/survey readiness and QA audits, including detailed corrective action plans.
• Manages HHCAHPS distribution in SHP.
• Leads emergency preparedness planning, activation of EPP, post-activation reporting, and ongoing event monitoring.

Requirements

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• 3-5 years of experience as an RN.
• Experience managing RN services in the home, community, or clinic is preferred.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with an automobile insured in accordance with state/organization requirements and in good working order.
• Ability to travel to all business locations.

Pay

$78,000–$84,000 DOE.

Schedule

Monday–Friday, 8:00 AM–5:00 PM.
