Clinical Manager position based in San Diego, CA. Full-time, in-office role with a schedule of Monday through Friday, 8 AM to 5 PM.

About the Role

Provide oversight of all patient care services and personnel. This includes coordination and management of patient care, supervision and coaching of clinical staff, CHAP/survey readiness, QAPI, patient satisfaction surveys, and complaint reviews.

Responsibilities

  • Supervise day-to-day clinical and office operations.
  • Ensure availability to staff and patients during hours of operation.
  • Ensure services comply with professional standards, agency policies, and state/federal regulations.
  • Serve as back-up support for PCM and CSS roles; lead in the absence of the Territory Administrator.
  • Coordinate referrals and patient care with an interdisciplinary team approach.
  • Develop, implement, and update individualized plans of care.
  • Evaluate the quality of clinical care and ensure continual patient needs assessment.
  • Collaborate with clinicians and physicians regarding patient transfers or discharges.
  • Review discharge assessment data, edit/lock discharge OASIS, and manage discipline-only discharges.
  • Own and participate in QAPI, QA, and performance improvement activities.
  • Review and investigate incidents, analyze trends, and implement corrective action plans.
  • Manage accreditation/survey readiness and QA audits, including detailed corrective action plans.
  • Oversee HHCAHPS distribution in SHP.
  • Lead emergency preparedness planning, activation, post-activation reporting, and ongoing event monitoring.

Requirements

  • Graduate from an approved school of professional nursing.
  • Current RN license in the state of agency operation.
  • 3-5 years of experience as a Registered Nurse.
  • Experience managing RN services in home health, community, or clinic settings preferred.
  • Valid CPR certification.
  • Licensed driver with an insured, well-maintained automobile.
  • Ability to travel to all business locations.

Pay

Salary range: $110,000 - $135,000 per year, based on experience.

Schedule

Monday through Friday, 8 AM to 5 PM (in-office).
