Responsibilities

  • Provide oversight of all personnel and patient care services
  • Monitor compliance, quality, and safe delivery of services by following clinical processes
  • Operations:
    • Ensure Start of Care compliance by the clinical team and focus on Outcome Monitors as assigned
    • Participate in all liaison/administrative on-call rotation and patient staffing as needed
    • Process complaint workflow, report staffing trends, neglect, abuse, or theft to Administrator for resolution and follow-up
  • Patient Care:
    • Complete patient visits as needed and assist with any patient issues
  • Budget:
    • Participate in managing cost of goods, monitoring supply costs, overtime, staffing, bonuses, and visits per episode
  • Hiring and Training:
    • Select and onboard staff
  • Collaboration:
    • Collaborate with intake, providing daily direction, and managing referral volume based on staffing availability
    • Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings, and families
  • Supervision:
    • Provide oversight for liaison and clinical staff
    • Ensure timeliness of completion of work/charting and identify education needs for the team
    • Manage on-call, PTO, call-offs, and daily/weekly productivity review to ensure proper staffing coverage
  • Minimize Patient Risk:
    • Ensure quality and safe delivery of home health services by the clinical team
  • Policies:
    • Review clinical documentation and maintain current knowledge of all Medicare/Federal/Insurance/Company and Hospital guidelines, policies, and protocol

Requirements

  • Current RN or PT license in the state of employment
  • Minimum of three (3) years of experience as an RN or PT in an acute care setting
  • Valid driver’s license and auto insurance in your name as a driver
  • Capable of all physical demands

Pay

$10,000 sign-on bonus

Schedule

Monday - Friday, 8:15am - 5:00pm
