Position Overview

Under the supervision of the Network Director, the Registered Nurse (RN) promotes the physical and mental well-being of people with disabilities and/or mental illness receiving support and services in the Behavioral Health Stabilization Homes Program. The RN collaborates with the support team to oversee medical services, including medication administration, ensuring compliance with regulatory requirements while addressing the unique needs of each individual. The RN empowers people to take personal responsibility for their health while ensuring their overall well-being.

Essential Functions

  • Conducting health and medical reviews/observations to complete required agency and regulatory assessments, including:

    • Health Risk Screening Tool: Administered during admission and annually thereafter.
    • Nursing Assessment: Conducted annually and updated as needed based on medical status.
    • Self-Administration of Medication Assessment (SAMA): Conducted at least annually and reviewed quarterly for individuals with co-occurring intellectual/developmental disabilities and mental illness.
    • Insulin Administration Training – Identify/develop and provide staff training on the safe administration of insulin.
  • Participating as a member of an individual’s support team in the person-centered planning process to ensure medical and healthcare needs are met.

  • Promoting self-care activities, including self-medication training and administration when appropriate.

  • Providing specialized instruction, education, and training to individuals and support staff regarding medical diagnoses and healthcare needs.

  • Available after hours for medication or emergency-related inquiries.

  • Consulting with team members on medical needs and coordinating follow-ups after medical appointments, lab work, and hospitalizations.

  • Coordinating care with clinics, physicians, and hospitals as assigned by the Network Director.

  • Communicating with parents/guardians regarding illness, injury, or changes in healthcare needs as assigned by the Network Director.

  • Overseeing medication and medical needs upon admission, post-hospital discharge, and as needed based on prescribed medications and health conditions.

  • Delegating medication administration tasks to non-licensed staff and providing regular on-site monitoring of tasks.

  • Providing initial and annual retraining on medication administration in compliance with Title 59 Part 116 of the Illinois Administrative Code.

  • Reviewing, updating, and maintaining accurate Physician Order Sheets (POS) and Medication Administration Records (MAR) in collaboration with medical professionals and pharmacies to ensure regulatory compliance.

  • Reviewing completed MARs at least monthly and taking appropriate follow-up actions, including timely notification of the Network Director regarding any concerns.

  • Reviewing and addressing injury, illness, and medication error reports, ensuring timely reporting to the Network Director.

  • Documenting direct services and instruction provided, ensuring compliance with agency policies and regulatory requirements.

  • Assisting with agency-wide training, including health and safety classes, as assigned by the Network Director.

  • Providing medication administration delegation support during scheduled RN vacations and other agency needs.

  • Assisting in the orientation of new nursing staff as needed.

  • Taking responsibility for professional development and continued growth.

  • Participating in agency training activities as assigned.

  • Engaging in agency-wide nursing and medical meetings.

  • Assisting in Behavioral Health crises including physical management.

Skills And Abilities

  • Ability to make independent decisions and solve problems within the scope of licensure and credentials.

  • Strong interpersonal skills with the ability to build rapport with staff, program participants, family members, and external stakeholders.

  • Effective communication skills, both verbal and written, to convey policies, procedures, and medical information clearly.

  • Ability to maintain compliance with agency policies, procedures, and regulatory standards.

  • Competency in reading, analyzing, and interpreting medical data, research, technical procedures, and regulations.

  • Proficiency in calculating medication dosages accurately.

Physical Demands

  • Ability to lift up to 40 pounds unassisted.

  • Ability to push and pull up to 75 pounds.

  • Ability to perform deep knee squats and push wheelchairs up to 200 pounds.

  • Ability to stand or sit for prolonged periods.

  • Capability to operate motor vehicles.

Qualifications

  • Minimum of an Associate’s Degree in Nursing (ADN) and an active Registered Professional Nurse license in the State of Illinois.

  • Successful completion of the Illinois Department of Human Services (DHS) Nurse-Trainer Training Program.

  • Preferably, at least one year of experience in nursing, with some exposure to individuals with intellectual/developmental disabilities and mental illness.
