About the role

VNA Health Group is a movement dedicated to transforming healthcare by empowering nurses and clinicians to provide exceptional care in patients' homes. We are seeking a Registered Nurse (RN) to join our team in Jersey City, NJ.

Responsibilities

• Assess patients with complex health needs and develop personalized care plans.
• Guide and educate patients and families through their healthcare journey.
• Collaborate with a team of VNA clinicians and external providers to ensure seamless care.
• Document changes in patient conditions and update care plans to reflect progress and needs.

Qualifications

• Bachelor’s degree in nursing preferred.
• At least 1 year of clinical experience as a registered nurse, with bonus points for homecare experience.
• Strong verbal and written communication skills.
• Proficiency with EMR systems and digital tools for enhanced patient care.
• A reliable car and valid driver’s license for travel to patient homes.

Pay & Benefits

The starting salary range for this position is USD $43.00 to USD $51.00 per hour. Additionally, VNA Health Group offers a $2500 sign-on bonus.

Physical Demands/Working Conditions

Flexibility to work both indoors and outdoors, using clinical equipment, and comfortable using various digital tools.
