Registered Nurse (RN) / Licensed Practical Nurse (LPN) - Long Term Care

About the Role

Provides comprehensive nursing care to residents, including developing care plans, administering treatments, and supporting physical, emotional, psychological, and spiritual needs. Collaborates with healthcare teams, reviews prescriptions, and communicates care objectives to ensure quality resident care.

Responsibilities

• Review physician prescriptions and coordinate with healthcare providers.
• Develop and modify nursing care plans based on resident assessments and changing needs.
• Educate residents and families on health conditions, medications, and self-care.
• Maintain a safe, clean environment adhering to infection control and safety protocols.
• Manage resident admissions, discharges, and transfers.
• Supervise and coordinate workflow of nursing staff and assistants.
• Ensure proper operation and stock of nursing equipment and supplies.
• Maintain documentation and evaluate care outcomes for continuous improvement.

Requirements

• Active RN license in the state of practice.
• Effective communication, visual acuity, and the ability to work collaboratively.
• Preferred 1-3 years of clinical or long-term care experience.
• Commitment to compassionate, patient-centered care and adherence to safety standards.
