Registered Nurse (RN) / Licensed Practical Nurse (LPN) | Long Term Care

About the Role

Providing professional nursing care to residents, including assessments, care planning, and evaluations within the scope of practice. Collaborates with the healthcare team, reviews physician prescriptions, and develops personalized care plans. Educates residents and families on health conditions, medications, and self-care. Maintains a safe environment by adhering to infection control, medication, and safety protocols.

Responsibilities

• Coordinate resident admissions, discharges, and transfers.
• Supervise and direct workflow of nursing staff and assistants.
• Ensure supplies and equipment are maintained and available.
• Evaluate and modify care plans based on resident needs and outcomes.
• Maintain compliance with safety, legal, and organizational standards.
• Provide emotional, psychological, and spiritual support to residents and families.

Requirements

• Active license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of practice.
• Effective communication and visual acuity.
• Preferred 1-3 years of clinical or long-term care experience.
