Registered Nurse (RN) - Hospice Outpatient, FT, Days

We are seeking a registered nurse to provide compassionate hospice and palliative care to terminally ill patients of all ages, supporting both patients and their families. Key Responsibilities Include Assessing patients' physiological, psychosocial, developmental, and environmental needs to develop individualized care plans in collaboration with the interdisciplinary team.Performing thorough physical, pain, and symptom assessments, and documenting findings clearly and promptly.Monitoring and updating care plans based on patient status and family needs.Providing patient and family education, supporting emotional and spiritual needs, and facilitating end-of-life planning.Ensuring safe, effective nursing interventions, utilizing standing orders, and adhering to regulatory and organizational policies.Overseeing CNA interventions and supervising team activities to maintain high standards of care.Participating in team meetings, quality improvement initiatives, and community education on end-of-life issues.Supporting bereavement follow-up and contributing to team-based care planning and case discussions. Qualifications And Skills Required Associate degree in Nursing; BSN preferred.Current RN license recognized in the state of practice.At least one year of nursing experience, preferably in hospice or home care; or completion of a nurse residency program.Strong critical thinking, problem-solving, communication, and cultural competency skills.Valid driver's license and acceptable driving record. This role involves providing holistic, patient-centered care, collaborating with team members, and supporting families through end-of-life processes, with a focus on compassionate service and clinical excellence.
